We provide diabetes services to individuals diagnosed with diabetes or individuals and professionals with interest in diabetes care.


Our services are individualized to meet the needs of our clients. Our services include:



We provide high quality individualized (one-on-one) diabetes self-management training based on the client’s assessed  needs and choice. We also provide Group Classes. Clients participating in our group classes also meet one-on-one with the nurse practitioner/diabetes educator for an initial consultation and assessment. The assessment helps us better understand your needs.

Depending on your assessed needs, our comprehensive (full class) consists of four (2 hours each) sessions.

We address the following:

  • General information about diabetes, and blood glucose (blood sugar) control
  • Healthy Eating and Carbohydrate counting
  • Physical activity and why it is important o your health and diabetes
  • How to take your medications and insulin (if needed) properly
  • Blood glucose monitoring and how to use the information to improve your diabetes
  • How to recognize complications like low blood sugar (hypoglycemia) and high blood sugar (hyperglycemia), and how to prevent or manage them
  • How to prevent long term complications
  • Coping with diabetes
  • Gestational (pregnancy) diabetes
  • Resources that are available to you

At the end of the classes, a report is sent to your healthcare provider, to ensure continuity of care and good clinical outcome.The program also provide ongoing telephonic support to help you manage your disease.




Diabetes is a lifelong disease that can be controlled with good management.

We partner with individuals and healthcare providers based on agreed time intervals to ensure a quality outcome. The service involves the following:

  • An initial comprehensive face-to-face assessment to identify diabetes related conditions that put the individual at risk for complications.
  • A collaborative care plan development with the individual based on assessment and their personal diabetes related needs and goal.
  • Periodic one-on-one contact with the individual, to provide self-management education and re-inforcement and support.
  • Scheduled reassessment and evaluation of condition and outcome
  • We also serve as liaison between the individual and healthcare providers providing referrals to improve follow up care.


Diabetes Concepts, LLC provides a range of consulting services that encompass:

  • Institutional needs assessment; policy and procedure development and update.      
  • Professional education/seminars
  • Product development consultations
  • Patient education records development


The kidney disease education is a six (1 hour each) classes that was designed by the National Kidney Foundation. The classes are for individuals that have been diagnosed with Chronic Kidney Disease (CKD), Stage four.  

These classes provide you with information to help you make informed-decision about your health. You will learn about:

  • Chronic kidney disease and how it affects your body
  • Available treatment choices for kidney failure
  • Kidney transplant
  • Hemodialysis
  • Peritoneal Dialysis
  • Your choice to have no treatment
  • What you can do to live with kidney disease

Classes are provided by a nurse practitioner who is also a certified diabetes educator with years of experience working with individuals on hemodialysis and peritoneal dialysis and individuals with kidney transplant.

 The classes are provided on a One-on-One or Group basis depending on your need.
